A young man was admitted ten times, with six abscondings, over thirty-four months. He was prescribed seven antipsychotic agents, lithium, and long-term benzodiazepines. His treating team's own genetic testing recorded him as a rapid metaboliser.
Pathology records are not released. Plasma drug concentration records are withheld from the patient.
What the record shows
- Ten admissions and six abscondings between September 2023 and August 2026.
- Seven antipsychotic agents — aripiprazole, brexpiprazole, paliperidone, haloperidol, olanzapine, zuclopenthixol, clozapine — plus lithium, benzodiazepines and anticholinergics.
- Discharged with five benzodiazepine tablets without supervision or explanation to Guardian. The first discharge summary set out a taper: six months' stabilisation, then a slow reduction with GP support. Within four weeks he was on a long-acting injectable. No taper was ever begun.
- A seizure at 400 mg of clozapine in March 2026, and an 800 mg double-dose error four days later.
- Records were refused for eleven months across five statutory requests. Latest response implied that the records are obstructed because regulatory agencies are investigating the complaints. A Queensland health service released 210 pages in full, on one request, free of charge, in a single decision.
